If help is needed, push your Lifeline button and a Lifeline Personal Response Associate will answer your call.

If you require help, give the Lifeline Associate the wrong password or do not speak at all. If you are ok or if you pushed your button by accident, quote password “Pineapple”.

If help is not needed, end the appointment by texting “Left” to Lifeline Text number.

Repeat steps 2 and 6 at every new appointment.

*Device must be used as directed. Button must be worn, adequately charged, and within range of a third-party cellular network. Availability of network not guaranteed. Tracking signal sent approx. every 10 minutes. If an on-site address was not provided, the Lifeline Response Associate will look up the GPS coordinates for an approximate location and then ask over the two-way speaker for the address.

Alarm Monitoring Consent

I, the undersigned, agree and acknowledge that Lifeline of Canada Holdings Ltd. (“Lifeline”) has been retained by Ontario Personal Support Worker Association (the “Organization”) to provide alarm monitoring services (the “Monitoring Services”) and related equipment, notably the Lifeline Mobile device (the “Equipment”).

1. In order for Lifeline to be able to perform its role and provide the Monitoring Services, the Organization may share with Lifeline certain of my personal information, including without limitation my name, telephone number and location information when receiving the Monitoring Services. In addition, for the purposes of performing the Monitoring Services, Lifeline may share my personal information, including any details about any Monitoring Services event Lifeline responds to, with emergency responders, the Organization, and other representatives of Lifeline, including its affiliates. Such personal information will be used and protected by Lifeline in accordance with applicable privacy laws and Lifeline’s privacy policy (available online at www.lifeline.ca). I understand that all telephone conversations between Lifeline and me may be recorded, and I hereby consent to such recording.

2. I consent to be contacted by Lifeline to receive electronic messages from Lifeline and I have been advised of the technical service limitations associated with the Monitoring Services and Equipment including the limitations of wireless equipment, wireless networks, SMS text messaging, and location tracking, and understand the potential impact that these service limitations may have on any Monitoring Services.

3. I acknowledge that I must have a smartphone that can send and receive SMS texts and data communication in order to provide exact CheckIn location information and upon exit of the address send an SMS or push the Mobile Help Button to speak with the Lifeline Response Centre to notify them I have left the premises, I will be required to provide my Safe Word. Lifeline is not responsible for any access, SMS text, data, or usage charges or any other charges in connection with any such smartphone and the Monitoring Services.

4. My role and responsibilities in connection with the Monitoring Services have been explained to me, and I agree to them, including that: (1) if Lifeline contacts me in response to an Equipment alarm, I must provide Lifeline with the “Safe Word” as submitted on my Intake Form (CarePlan), if I do not require any emergency assistance; (2) if I do not provide Lifeline with the Safe Word, Lifeline may dispatch emergency services to my location, in its discretion.

5. I acknowledge and agree that Lifeline offers no guarantees of specific performance levels for the Monitoring Services, the Equipment and wireless communications system and connectivity necessary to perform the Services, and that Lifeline does not guarantee that Lifeline can always track or otherwise identify my location, nor that calls to emergency services, or the Lifeline monitoring response center will be completed. I acknowledge that it is required that I send an SMS message of my exact location prior to entering. In the event that I do not submit an SMS I understand that Lifeline may not be able to identify my specific location ie certain apartment or unit number in the event of an alarm.

6. Lifeline may locate the Equipment and contact me, and/or emergency responders if, as applicable (i) I do not respond to the Equipment as required or within the required time; or (ii) an alarm is triggered on the Equipment provided to me. Lifeline is not responsible for the actions or inactions (including the promptness, sufficiency, or adequacy) of any emergency responder, or other third party that is notified, directly or indirectly, by Lifeline (on behalf of the Organization).

7. I acknowledge that I have received a copy of, read and understand the Lifeline Equipment manual and instructions for use (the “Instructions”), and I agree to follow the Instructions as applicable to the Monitoring Services, including at all times ensure that Equipment is properly maintained and operated in accordance with the Manuals, including ensuring that Equipment batteries are fully charged.

8. I understand that any cord worn around the neck may pose a strangulation risk; and agree not to tie a knot in the Equipment neck cord, or otherwise modify the Equipment in any way.

9. I acknowledge and agree that it is only when the Equipment is activated that Lifeline receives the Equipment’s co-ordinates using global positioning satellites. Lifeline cannot determine the Equipment’s location without this activation request for Service. At the Equipment installation, ensure that the Equipment has transmitted a successful signal to Lifeline’s response center, and that the signal has been acknowledged by a Lifeline Representative, and thereafter, ensure that the Equipment continues to transmit a signal to the Lifeline response center in accordance with the Manuals. I acknowledge that the Equipment is intended for use in Canada only and that I am solely responsible to ensure that all information provided to Lifeline is accurate and up-to-date at all times, including sending a SMS of the full address prior to entering an appointment and understands that any inaccuracy to such information may result in delay or failure of the Services provided hereunder.

10. I acknowledge that implantable cardiac devices such as a pacemaker or defibrillator may not be compatible with all models of Mobile Help buttons. Lifeline products are tested, as are other cellular and wireless communications products licensed in North America. Individuals with pacemakers should review their pacemaker materials regarding interaction with cell phones and take the same precautions the materials recommend for this device. Consult with your healthcare professional.

11. I acknowledge that the Fall Detection feature as outlined in the Instructions is disabled and will not detect a fall event and will not automatically send a signal to the Response Centre.

By acknowledging the Alarm Monitoring Constent, I agree that Lifeline and its representatives have no liability to me, my heirs or estate, for any death, personal injury, property damage or any special, consequential, indirect, incidental or punitive damages (including any loss of data, such as personal information) in connection with the Monitoring Services. Specifically, Lifeline does not warrant that the Monitoring Services will prevent any injury, death, medical condition, loss, or damage.
